I, hereby, give my child permission to participate in Fortunato Karate’s 2011 Summer Super Camp. I understand that great care will be taken to provide a safe training atmosphere. However, I realize that karate, jiu-jitsu, board breaking and weapon training, just like any other physical activity; brings about some level of risk of injury. I acknowledge this risk and I will hold Fortunato Karate, it’s instructors, helpers and volunteers free from any and all liability.
